COMMUNICABLE DISEASE SURVEILLANCE IN CROATIA

    Communicable disease case notification in August

S a l m o n e l l o s i s . There were more cases reported  in August than in July (412:381) which was also more than in August last year,  but without larger clusters. Highest numbers are in Rijeka (36) and Novi Zagreb (29).

H e p a t i t i s  A (epidemic jaundice). In August 7 cases were reported,  4  of them from Cres-Losinj. This was slightly more than in July (4) and last August (1).

M o r b i l l i  (measles). One report of a case from previous month came in August from Zagreb Medvescak. It was not virologically confirmed.

R u b e l l a. No cases in August.

M e n i n g i t i s   v i r o s a (aseptic). A slight summer increase is observed (41:36), but incidence has been higher in August last year (132).

T B E  (tick born encephalitis). This summer low incidence is seen,  with only 3 cases in August (8 in July). Last year intensity was similar (2).

L y m e   b o r r e l I i o s i s. Usual seasonal increase occurs, following a pick of tick activity (94: 80). In August last year intensity was lower (46).

B r u c e l l o s i s. One case reported in August from Karlovac area. Last year there were no cases in August.

      No reports in August from:

Nova Gradiska, Otocac, Buzet, Gracac, Sinj, Vis, Lastovo, Metkovic, Jastrebarsko and Zagreb Susedgrad. Total: 10 out of 113 epidemiological districts.

      Epidemic outbreaks

Following outbreaks were reported in August:

Note: in all outbreaks mentioned here, epidemiological investigation and necessary measures were undertaken, preventing thus other potential cases.

Opatija - salmonellosis (S. enteritidis) in a kindergarten on 30.5. 2008. There were 7 diseased and 4 more simptomless carriers found out of 53 exposed persons. Vehicle not found.

Krk – histamine poisoning after a fish meal on 5.8. 2008, with 4 diseased (all exposed). Raw fish was kept outside refrigerator for a day.

Varazdin - scabies in a social institution. On 25.8. 2008 there were 8 infested out of 180 exposed persons.


NEWS AND COMMENTS

       Composition of influenza vaccine for the season 2008/2009

In the forthcoming season the vaccine against influenza will consist itself of the following viral strains (antigens) according to the WHO recommendation published in the WHO Weekly epidemiological record No 9 /2008:

A/H1N1/Brisbane 2007 like

A/H3N2/ Brisbane 2007 like

B/Florida 2006 like

It can be concluded that above basic N and H antigens were already seen, however different to a some extent as every year (could be recognized from their new names), this giving influenza always again a new epidemic potential. However, no elements suggest a new pandemic this season up to now.

       Results of systematic testing of pregnant women for hepatitis B in 2007

Since 1999 in Croatia systematic testing of all pregnant women is in place, in order to protect timely the newborns of HbsAg positive mothers against hepatitis B. Every year this measure is checked by the analysis of annual reports of all maternity wards in the country (34) about testing pregnant women. Such analysis was done this year too, for the previous year 2007. The results are satisfying as in pervious years (see ENEWS 8/2007). A high coverage is achieved (99,4%), i.e. 41544 tested pregnant women in the total of 41777 labors registered.  Among them 104 HbsAg positives were detected (0,25%), proportion varying between 0 % and 0.64 % in different maternity wards or districts. This is less than previous year (0,34%) and very probably is a positive consequence of systematic vaccination starting also in 1999,  and covering now all persons between 12 and 20 years of age (that include also a portion of nowadays young pregnant women). The table 1 shows the results according to particular maternity ward and in the total.

The total of 104 pregnant women had been found to have active infection. They gave birth to 104 newborns which were promptly protected.
