COMMUNICABLE DISEASE SURVEILLANCE IN CROATIA

    Communicable disease case notification in January

S a l m o n e l l o s i s.  Incidence is lower in January than in December (183:297). Small clusters are seen in Rijeka (12), Split (13), Velika Gorica (19) and Novi Zagreb (13).

H e p a t i t i s   A (epidemic jaundice). Low intensity continues: Jan: 1, December 1, January last year: 2.

 P e r t u s s i s  (whooping cough). There were 8 cases registered in January, which was similar to previous month (9) and to January last year (7).

M o r b i l l i  (measles). No reports in January

R u b e l l a . No reports in January

V a r i c e l l a (chicken pox).  Incidence raised in January as expected for the season (1973: 1659), but  in January last year  intensity was higher (2553).

S y p h i l i s. In January  4 cases were reported, slightly more than in December (1), equal to January last year (4).

M a l a r i a. One imported case (Ethiopia) was reported  in January from Karlovac.

I n f l u e n z a. Influenza season started in January and 866 cases were recorded up to the end of the month. In January last year the number was higher (2126).

C a m p y l o b a c t e r i o s i s. In accordance with the EU rules of disease monitoring, since the beginning of this year campylobacteriosis is monitored as a separate entity. In January there were 14 cases reported. Small cluster is seen in Varazdin (4).

      No reports in January from:

Cazma, Rovinj, Pag, Vis, Lastovo and Sv. Ivan Zelina. Total: 7 out of 113 epidemiological districts.

      Epidemic outbreaks

Following outbreaks were reported in January:

Note: in all outbreaks mentioned here, epidemiological investigation and necessary measures were undertaken, preventing thus other potential cases.

Karlovac – brucellosis in a village with  4 diseased registered between 8.4. and 22.10. 2008. Two of the diseased were owners of a sheep flock found affected by Brucella mellitensis , another two were veterinarians working on the control of disease in animals. Mode of  infection: contact with animals and their excreta, in 1 case it was by consumption of raw  milk.  

Dubrovnik – gastroenteritis in a group of construction workers on 9.10. 2008. There were2 diseased out of  17 exposed. Causative agent not found, laboratory tests negative. Mode of spread probably by food. Vehicle epidemiologically: backed pork and potato  meal.

Crikvenica – streptococcal sore throat in a settlement, mainly among preschool and school children. Between 3.11. and 12.12. 2008. there were 60 diseased.

Slatina – salmonella food poisoning (S. typhimurium) starting on 5.11. 2008. There were 3 diseases out of 6 family members.  Vehicle not found. Domestic eggs suspected but found negative for salmonella in the laboratory.

Cakovec – salmonella food poisoning (S. enteritidis) after a family celebration on 23.11. 2008. There were 6 diseased out of 13 exposed persons. Vehicle epidemiologically: home made cakes called "freezing wind"

Korenica – gastroenterits in a home for elderly. Causative agent not found. Laboratory examinations negative. Between 27.11. and 5.12. 2008. there were 25 diseased out of  70 exposed persons. Mode of spread: contact.

Rovinj – paralytic shellfish poisoning (PSP) on 16.12. 2008. in a group of persons eating a specialty  called "sea eggs" (organism of Microcosmus genus). There were 7 diseased having symptoms of general fatigue, vertigo, vision disturbances and orientation loss. All symptoms stopped spontaneously in two hours. Laboratory investigation sea eggs turned positive for PSP biotoxins.

Pakrac – viral gastroenteritis in an hospital. Causative agent: noro virus. Between 19.12. and 29.12. 2008 there were 10  diseased. Mode of spread: contact.


NEWS AND COMMENTS

       Completeness of reporting in 2008

Every year completeness of reporting is analyzed in order to asses the quality of the insight into actual epidemic situation in the country, and to improve it further. Analyzing reports on a monthly level, it can be seen that like in previous years, sporadic absence from the monitoring system had been recorded. The table shows how many epidemiological districts were missing during particular month of 2008.

This means that every month,  94% of all districts were continuously represented in the national surveillance system  (total number of districts is 113),  which can be assessed as favorable. However comparing to previous year with 6 (5,5%) situation is similar (see ENEWS no 1 /2008). One month without reports during the year is registered in 15 different districts. Two remote, less populated island districts  were absent for ten and eleven month respectively. Without any absence during all 12 months  were 79 epidemiological districts.

The total number of registered disease cases (64452) does not differ from previous year (64039). Recently, primary health care system became computerized and included into the national network. This can facilitate the obligation of prompt active reporting (in 24 h) of all cases of communicable diseases from the national list, to the filed epidemiology services.

       Malaria in Croatia 2008

Croatia is malaria free since 1954 when the last autochthonous case had been registered as a result of successful national antimalarial campaign. However certain number of imported ceases is registered each year, in persons that acquire the disease in some of world malaric areas returning home for treatment or getting ill after their return.

In 2008 there were 6 such cases with 4 different persons included i.e. the two  experienced a relapse of the disease. In previous years number of cases was slightly higher. Some data about the patients obtained through interviews with diseased persons are given in the table.

All diseased lived for longer period in mentioned countries for business or research reasons. No sailors among diseased, while this category was dominating in previous years.
