COMMUNICABLE DISEASE SURVEILLANCE IN CROATIA

    Communicable disease case notification in February

S a l m o n e l l o s i s.  Number in February is lower than in January, but higher than in February last year. The only cluster is seen in Pula (15).

H e p a t i t i s   A (epidemic jaundice). A favorable low incidence situation continues, with only 2 cases registered in February.

S t r e p t o c o c c o s i s. (streptococcal sore throat & scarlatina). Seasonal increase of incidence is observed, intensity higher than in February last year. Total number in February was 1598 (1045 sore throats and 553 scarlet fevers), which was more than in previous month (1021) and in February last year (1020).

T e t a n u s. One case reported from Djurdjevac,  in an old unvaccinated person.

M o r bi l l i (measles). No reports in February.

R u b e l l a. No reports in February.

M e n i n g i t i s   e p i d e m i c a. In February there were slightly less cases reported than in January (8:19) as well as  in February last year (10). No clusters.

T r i c h i n e l l o s i s. There were 19 cases recorded in February after a longer period without trichinellosis. Cases were reported from Djakovo (5), Vinkovci (5), Sinj (6), Split (1) and Ivanec (2).

M a l a r i a. In February 1 imported case (Sierra Leone) was reported from Zagreb Susedgrad.

I n f l u e n z a. Starting in January, the wave developed in February with a total of 33593 registered cases.

C r e u t z f e l d t  – J a k o b disease. One case reported from Bjelovar. l Investigation revealed that that was not a case of vCJD i.e. new variant connected with so called mad cow disease.

      No reports in February from:

Novalja, Vis, Lastovo. Total: 3 epidemiological districts out of 113.

      Epidemic outbreaks

Following outbreaks were reported in February:

Note: in all outbreaks mentioned here, epidemiological investigation and necessary measures were undertaken, preventing thus other potential cases.

Krapina – scabies in a social institution. On 14.7. 2008. there were 40 infested persons found out of 350 exposed.

Dubrovnik – bacillary dysentery (Sh. sonnei) among members of two connected families on 9.9. 2008. There were  3 diseased out of 11 exposed. Source not found.

Dubrovnik – viral gastroenteritis (noro virus) in a hospital ward. Between 21.11. and 27.11. 2008  there were 14 diseased registered (patients and staff). Mode of spread: contact.

Delnice – gastroenteritis in an home for elderly persons. Between 15.12. and 18.12. 2008. the total of 24 cases out of 58 exposed persons were recorded. Causative agent: noro virus. Mode of spread most probably by food,  but vehicle not detected.

Topusko – gastroenteritis in an health care institution, caused by noro virus. Between 27.12. 2008 and 12.1. 2009 there were 14 diseased. Source found with certainty.

Split – viral enterocolitis in an home for elderly. Causative agent: noro virus. Between 7.1. and 30.1. 2009. there were 97 diseased out of 640 exposed persons. Mode of spread: mainly contact.


NEWS AND COMMENTS

       Influenza 2009.

As expected, a steep increase of incidence occurred in February all over the country. The laboratory of the National influenza center of the Croatian national institute of public health is reporting influenza virus A/H3N2/ as a causative agent of  cases in February.

       Pneumonia in 2008

In 2008 there were 4950 pneumonia cases reported, which was slightly less than last year (5431). The purpose of reporting pneumonia cases (so called syndromic reporting) in spite of the fact that the majority is not transmissible among humans, and usually no control measures could be applied, is to enable noticing clusters of cases which could point  to some etiological situations where intervention is possible and useful, like in legionnaires diseases, Q fever, mycoplasma pneumonias etc. Pneumonias diagnostically determined as Q fever or legionellosis cases are then monitored separately. Among other pneumonia cases causative agent usually is not detected. In 2008 causative agent is noted in 53 of them. The Table 1 shows which agents were detected in 2008.  Comparing to previous years the structure is similar. Difference is seen only with Mycoplasma which incidence  and relative share may fluctuate from year to year.
