COMMUNICABLE DISEASE SURVEILLANCE IN CROATIA

•    Communicable disease case notification in May

S a l m o n e l l o s i s. Number of cases registered in May was higher than in April (2003:121). Last year intensity was slightly lower (196). Small clusters are seen in Koprivnica (20), Virovitica (12), Rijeka  (12) and Zagreb Tresnjekva (12).

M o r bi l l i (measles). No reports in May

R u b e l l a. No reports in May

P a r o t I t I s epidemica (mumps). Only 2 cases were registered in May, 8 last year in the  same month. 

S y p h i l i s. There were 5 cases reported in May, 3 of them in Zadar area (Benkovac:1, Biograd:1, Zadar : 1). This is similar to previous month (6), while in May last year only 2 cases were recorded.

M e n i n g i t i s   v i r o s a (aseptic). Beginning of seasonal raise registered  (14:7). In May last year number was similar (13). No clusters.

Q  f e v e r. There were 2 cases reported in May (Knin 1, Ploce 1), similarly to April (3), slightly less than in May last year (4).

I n f l u e n z a. In May, last 186 cases were reported (4176 cases in April), and the 2008/2009 outbreak stopped. Total of 54080 cases were recorded during current  outbreak. That was insignificantly more than in previous year (53688).

•      No reports in May from:

Garesnica, Vojnic, Opatija, Rovinj, Gracac, Makarska, Solin, Vis and Lastovo. Total: 8 out of 113 epidemiological districts.

•      Epidemic outbreaks

Following outbreaks were reported in May:

Note: in all outbreaks mentioned here, epidemiological investigation and necessary measures were undertaken, preventing thus other potential cases.

Dubrovnik – streptococcal sore throat in a kindergarten. There were 4 diseased out of 18  exposed between 12.4. and 23.4. 2009.

Donja Stubica – gastroenteritis in a groups of school children on excursion, settled in a hotel. There were 12 diseased out of 108 exposed. Causative agent not found,  analyses negative. Mode of spread not detected. Epidemiological investigation in the hotel did not reveal hygienic week points; personnel found pathogenic bacteria negative

Makarska  - salmonella food poisoning (S. typhimurium) in a group of  workers having their hot meal in a field. Vehicle epidemiologically: backed pork meat, kept outside refrigerator in a pick up van for several hours. Between 6.5. and 11.5. 2009 there were 3 diseased out of 70 exposed.

Samobor - salmonella food poisoning (S. enteritidis) on 17.5. 2009 in a group of construction workers after a meal of pork goulash with pasta, brought from a near restaurant. No samples left for analyses.


NEWS AND COMMENTS

•       Influenza season 2008/2009

Seasonal influenza wave stopped completely in May. A total of 54080 cases  was registered in that outbreak, insignificantly more than last year (53688). The virological laboratory of the National influenza center of the Croatian national institute of public health detected  viruses A/H3N2/ and B in the circulation, type B appearing in the second half of the outbreak wave. The graph shows the outbreak development by months.

•       Wild polio virus returned to some countries in Africa

The WHO Weekly epidemiological record No 16 of 17th April 2009 publishes the article titled Resurgence of wild poliovirus types 1 and 3 in 15 African countries, January 2008–March 2009.  It can be learned from the article that in 2008 and first three months of 2009, wild polioviruses of type 1 and 3 (WPV1 and WPV 3) had been imported  to several African countries previously succeeding in interrupting wild virus transmission. A total of 32 such events was recorded, producing 96 diseased with  poliomyelitis. Imports occurred mostly from African countries where transmission of wild virus is still not stopped, while a  part originated from wider Indian subcontinent area, where also  wild virus circulation still exists.

Ed. comment. Events like the described, that could have happened elsewhere too, together with overall polio situation in the world, explain why polio vaccination is kept in the  national Program of mass vaccination (like in other countries of the world) in spite of the fact that eradication was declared in Croatia and in the entire WHO European region since 2002, and why a high vaccination coverage achieved in the country should be maintained.

Beside that, that information reminds doctors dealing  with travel medicine, that every traveler to remote countries especially to areas mentioned, should be checked for polio vaccinal status and should be vaccinated  before a planned trip if necessary.
